    The President of the United States of America, in the name of Congress, takes pleasure in presenting the Medal of Honor to Colonel Lewis Addison Grant, United States Army, for extraordinary heroism on 3 May 1863, while serving with 5th Vermont Infantry, in action at Salem Heights, Virginia. Colonel Grant displayed personal gallantry and intrepidity in the management of his brigade and in leading it in the assault in which he was wounded.
