The President of the United States of America, in the name of Congress, takes pleasure in presenting the Medal of Honor to Private John W. Boutwell, United States Army, for extraordinary heroism on 2 April 1865, while serving with Company B, 18th New Hampshire Infantry, in action at Petersburg, Virginia. Private Boutwell brought off from the picket line, under heavy fire, a comrade who had been shot through both legs.
