    The President of the United States of America, in the name of Congress, takes pleasure in presenting the Medal of Honor to Sergeant Thomas D. Collins, Sr., United States Army, for extraordinary heroism on 15 May 1864, while serving with Company H, Sullivan County Regiment, 143d New York Infantry, in action at Resaca, Georgia. Sergeant Collins captured a regimental flag of the enemy.
